When you can hear, but can’t catch the words – what are you missing?

Some people are not missing sound. They are missing meaning.

“I can hear my wife talking from the next room. I just can’t tell what she said.”

I hear versions of this regularly. People know someone is speaking, but the words seem to disappear. How can both things be true?

Hearing sound and understanding speech are related, but they are not quite the same thing.

You might hear the neighbour’s pool pump, notice the rubbish truck coming down the street, or pick up small changes in the sound of an engine, yet still struggle to follow a conversation.

Recognising a familiar sound is one thing. Understanding speech is another.

Small differences between speech sounds carry important meaning. When those details are missed, or buried in background noise, you can hear a voice without being able to make out the words.

Try picturing a line of printed text with the tops and bottoms of the letters covered by strips of paper. You can still see parts of the words and recognise some of them. But reading takes more effort, and it becomes easier to mistake one word for another. That is rather like what conversation can feel like when some of the important details in speech are missing.

It often shows up in ordinary moments. At the pub, you hear the laughter but miss the punchline. At the bowls club or RSL, you follow the conversation until background noise and several people talking make it harder to keep up. On the phone, without facial expressions to help, even a familiar voice may become difficult to understand.

Here is something worth knowing about hearing assessments.

The familiar hearing test using beeps measures how softly you can detect different sounds. That is important information, but it does not always tell us how well you understand speech, particularly when other sounds are competing for your attention.

That is why a thorough assessment also considers speech understanding, listening in background noise, and the everyday situations where communication becomes difficult.

At A Better Ear, our clinical team works with this distinction every day. We want to understand not only whether you can hear someone speaking, but how easily you can follow what they are saying.

So if you often find yourself saying, “I heard you, but I didn’t catch that,” bring those words, and a few examples of when it happens, to your hearing assessment. They are not small details. They are valuable clues to understanding what is really going on.
