Forklift death sparks safety alert

A FATAL forklift incident at a Sheldon quarry has triggered an urgent safety alert, with Queensland’s resources safety watchdog warning industry to review critical risk controls.

A worker died on April 20 after the forklift he was operating rolled onto its side while travelling on a ramp.

In response, Resources Safety and Health Queensland (RSHQ) has issued a Safety Alert to the mining and quarrying sector, urging operators to immediately reassess how forklift risks are managed.

RSHQ Operations General Manager Donna Heelan said investigators were already on the ground.

“Our team is on site and are conducting a thorough investigation,” she said.

The alert is intended to provide early guidance to industry while the full investigation continues, with a focus on preventing similar incidents.

An RSHQ spokesperson said the regulator’s immediate priority was worker safety across the sector.

“Our thoughts are with the worker’s family, friends and colleagues during this difficult time,” the spokesperson said.

Authorities confirmed the site remains under investigation, with further findings and detailed safety advice to be released through the official Safety Alert.
